Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the rehab center, keeping dignity front and center
• Administer medications and treatments accurately and document thoroughly
• Monitor patient conditions and promptly report changes to the care team
• Collaborate with RNs, therapists, and support staff to deliver coordinated care plans
• Support activities of daily living and encourage resident independence when possible
• Educate residents and families with a kind, clear approach that makes healthcare feel human

Qualifications

• Current LPN license in New Mexico (or compact license where applicable)
• T-LPN experience accepted/preferred (per job posting)
• Current BLS (Basic Life Support) certification required
• Prior long-term care or rehab experience preferred — 1+ year recommended
• Strong communication skills and a team-first attitude

Education

• Completion of an accredited LPN/licensed practical nurse program or vocational nursing diploma

Knowledge & Skills

• Medication administration and accurate charting
• Wound care basics and infection prevention principles
• Familiarity with rehab nursing routines and care plans
• Good assessment skills and ability to escalate concerns appropriately
• Comfortable working overnight shifts with solid time-management skills
