Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the rehabilitation center
• Administer medications and treatments according to physician orders and facility protocols
• Monitor and document patient condition, responses to treatment, and progress toward goals
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Educate patients and families about care plans, medications, and discharge instructions
• Ensure accurate, timely charting and maintain confidentiality of patient information
• Respond promptly to patient needs and escalate changes in condition to appropriate clinicians

Qualifications

• Active LPN license in New Mexico (LPN). T-LPN candidates considered if applicable credentials are in place
• Current BLS/CPR certification required
• Minimum clinical experience in long-term care, rehab, or acute care preferred
• Up-to-date immunizations and TB screening per facility policy
• Strong communication skills and ability to work effectively in a team

Education

• Vocational nursing diploma, LPN program completion, or equivalent

Knowledge & Skills

• Solid clinical assessment and medication administration skills
• Familiarity with charting systems and basic documentation best practices
• Ability to prioritize care in a fast-paced environment
• Compassionate bedside manner and patient advocacy
• Basic wound care and rehabilitation support knowledge
