Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in the rehabilitation center
- Administer medications and treatments according to physician orders and facility protocols
- Monitor and document patient condition, responses to treatment, and progress toward goals
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Educate patients and families about care plans, medications, and discharge instructions
- Ensure accurate, timely charting and maintain confidentiality of patient information
- Respond promptly to patient needs and escalate changes in condition to appropriate clinicians
Qualifications
- Active LPN license in New Mexico (LPN). T-LPN candidates considered if applicable credentials are in place
- Current BLS/CPR certification required
- Minimum clinical experience in long-term care, rehab, or acute care preferred
- Up-to-date immunizations and TB screening per facility policy
- Strong communication skills and ability to work effectively in a team
Education
- Vocational nursing diploma, LPN program completion, or equivalent
Knowledge & Skills
- Solid clinical assessment and medication administration skills
- Familiarity with charting systems and basic documentation best practices
- Ability to prioritize care in a fast-paced environment
- Compassionate bedside manner and patient advocacy
- Basic wound care and rehabilitation support knowledge
