Roles & Responsibilities
- Provide compassionate, skilled nursing care to residents in a rehab setting
- Administer medications and treatments accurately and on time
- Monitor resident condition and communicate changes to the care team
- Document care activities clearly and timely in resident charts
- Collaborate with therapists and interdisciplinary team members to support recovery goals
Qualifications
- Current LPN license in New Mexico (required)
- Minimum relevant clinical experience preferred in rehab or long-term care
- Current BLS/CPR certification (or willing to obtain)
- Up-to-date immunizations and TB screening per facility policy
- Ability to work overnight shifts and adapt to a dynamic rehab environment
Education
- High school diploma or equivalent
- Completion of an accredited LPN/LVN program and active licensure
Knowledge & Skills
- Strong fundamentals in medication administration and wound care
- Good assessment skills and the ability to recognize changes in condition
- Comfortable with charting and following care plans
- Effective communication and teamwork in a multidisciplinary setting
- Time management and prioritization during busy shifts
