Roles & Responsibilities
- Provide direct patient care and skilled nursing within the rehab environment
- Administer medications and treatments accurately and safely
- Monitor patient status and communicate changes to the interdisciplinary team
- Document care, assessments, and interventions in the medical record
- Assist with patient mobility, activities of daily living, and rehab routines
- Support discharge planning and coordinate care with caregivers and families
- Maintain a safe, clean, and compassionate patient care environment
Qualifications
- Active LPN license in New Mexico (required)
- Current basic life support (BLS) certification preferred or willing to obtain
- Comfortable working overnight/NOC shifts
- Strong communication skills and ability to work within a rehab team
- Ability to pass standard background checks and health screenings
Education
- Completed accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Clinical nursing skills relevant to long-term care and rehabilitation
- Medication administration and documentation best practices
- Patient safety, infection control, and wound care basics
- Skilled at observing and reporting changes in patient condition
- Team collaboration and clear communication with clinicians and families
