Roles & Responsibilities
- Provide skilled nursing care to rehab patients, including medication administration and wound care
- Monitor and document patient vitals, responses to treatment, and condition changes accurately
- Collaborate with therapists and interdisciplinary team members to support individualized care plans
- Support patients with activities of daily living and mobility assistance as appropriate
- Communicate clearly with patients, families, and staff to ensure seamless transitions and high-quality care
- Assist with admissions, discharges, and timely charting to maintain unit flow and compliance
Qualifications
- Current LPN license in New Mexico (required)
- CPR/BLS certification (required or obtain upon hire)
- Experience in long-term care or rehabilitation nursing preferred
- Ability to work overnight NOC shifts and handle 12+ hour shift responsibilities
- Current immunizations and TB screening per facility policy
- Strong communication, teamwork, and patient-centered caregiving skills
Education
- LPN diploma or equivalent from an accredited nursing program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and rehab nursing principles
- Familiarity with electronic medical records and timely charting
- Ability to recognize and escalate changes in patient condition
- Team collaboration with PT/OT/Speech and restorative nursing staff
- Time management and prioritization in a fast-paced rehab setting
