Roles & Responsibilities
- Provide compassionate direct patient care to residents, focusing on dignity and comfort
- Administer medications and treatments accurately and document per facility protocol
- Monitor and record vital signs and report changes in condition promptly
- Collaborate with the interdisciplinary team to develop and follow individualized care plans
- Maintain accurate, timely patient records in the charting system
- Educate residents and families about care plans, medications, and health maintenance
Qualifications
- Active LPN license; RN candidates considered if applying for RN role
- Recent experience in long term care or skilled nursing is preferred
- Current BLS/CPR certification
- Strong communication skills and a team-oriented attitude
- Ability to pass facility background and health screenings
Education
- Completion of an accredited LPN program and high school diploma or equivalent
Knowledge & Skills
- Patient assessment and monitoring
- Medication administration and documentation
- Care planning and implementation in long term care settings
- Infection control practices and safety protocols
- Electronic health record documentation
