Roles & Responsibilities

• Provide compassionate direct patient care to residents in a long-term care setting
• Administer medications and treatments accurately and document per facility policy
• Perform timely assessments and monitor resident condition, reporting changes to the RN or provider
• Collaborate with the interdisciplinary care team to create and implement care plans
• Maintain accurate, timely charting in the electronic medical record
• Support residents and families with clear communication and empathy

Qualifications

• Valid LPN license (or eligible) for Maine — must be in good standing
• Current CPR/BLS certification (or able to obtain prior to start)
• Previous LPN experience in long-term care or similar setting preferred
• Ability to pass background checks and health screening per facility requirements

Education

• LPN diploma or certificate from an accredited program

Knowledge & Skills

• Strong skills in medication administration and basic nursing procedures
• Comfortable performing resident assessments, vital signs, and basic wound care
• Familiarity with charting and electronic medical records
• Good communication, teamwork, and time-management skills
• Ability to prioritize tasks in a fast-paced long-term care environment
