Roles & Responsibilities
- Provide compassionate direct patient care to residents in a long-term care setting
- Administer medications and treatments accurately and document per facility policy
- Perform timely assessments and monitor resident condition, reporting changes to the RN or provider
- Collaborate with the interdisciplinary care team to create and implement care plans
- Maintain accurate, timely charting in the electronic medical record
- Support residents and families with clear communication and empathy
Qualifications
- Valid LPN license (or eligible) for Maine — must be in good standing
- Current CPR/BLS certification (or able to obtain prior to start)
- Previous LPN experience in long-term care or similar setting preferred
- Ability to pass background checks and health screening per facility requirements
Education
- LPN diploma or certificate from an accredited program
Knowledge & Skills
- Strong skills in medication administration and basic nursing procedures
- Comfortable performing resident assessments, vital signs, and basic wound care
- Familiarity with charting and electronic medical records
- Good communication, teamwork, and time-management skills
- Ability to prioritize tasks in a fast-paced long-term care environment
