Roles & Responsibilities
- Coordinate and lead discharge planning and care coordination to streamline patient transitions
- Perform utilization review and determine medical necessity using evidence-based InterQual criteria
- Organize and participate in family meetings and collaborate on individualized treatment plans
- Arrange DME, SNF placements, and ambulance transport independently and efficiently
- Document thoroughly in Epic to ensure accurate, timely medical records and care plans
- Work with interdisciplinary teams (PT, RT, social services, IV teams) to optimize patient outcomes
- Support worker's compensation and disability case management activities when applicable
Qualifications
- Minimum 2 years acute hospital case management experience performing care coordination and discharge planning
- Charting in Epic is required
- Travel experience required — candidate must meet experience requirements to screen
- Ability to float between like units based on need
- Comfort managing patient ratios up to 1:20 on day shifts
- BLS (Basic Life Support) preferred
- Experience with HMOs/IPAs, prior authorizations, and SNF/DME arrangements
- Ability to provide documentation of current work history within the past 2 years (HARD STOP)
Knowledge & Skills
- Thorough understanding of discharge planning and care coordination best practices
- Familiarity with InterQual criteria, DRG, CPT and ICD-10 coding basics
- Knowledge of CMS, NCQA, HIPAA, OSHA and Joint Commission standards
- Experience with utilization review, prior authorizations, and benefits eligibility
- Strong communication, family meeting facilitation, and interdisciplinary collaboration skills
- Ability to perform data abstraction and determine medical necessity per guidelines
