Roles & Responsibilities

• Coordinate and lead discharge planning and care coordination to streamline patient transitions
• Perform utilization review and determine medical necessity using evidence-based InterQual criteria
• Organize and participate in family meetings and collaborate on individualized treatment plans
• Arrange DME, SNF placements, and ambulance transport independently and efficiently
• Document thoroughly in Epic to ensure accurate, timely medical records and care plans
• Work with interdisciplinary teams (PT, RT, social services, IV teams) to optimize patient outcomes
• Support worker's compensation and disability case management activities when applicable

Qualifications

• Minimum 2 years acute hospital case management experience performing care coordination and discharge planning
• Charting in Epic is required
• Travel experience required — candidate must meet experience requirements to screen
• Ability to float between like units based on need
• Comfort managing patient ratios up to 1:20 on day shifts
• BLS (Basic Life Support) preferred
• Experience with HMOs/IPAs, prior authorizations, and SNF/DME arrangements
• Ability to provide documentation of current work history within the past 2 years (HARD STOP)

Knowledge & Skills

• Thorough understanding of discharge planning and care coordination best practices
• Familiarity with InterQual criteria, DRG, CPT and ICD-10 coding basics
• Knowledge of CMS, NCQA, HIPAA, OSHA and Joint Commission standards
• Experience with utilization review, prior authorizations, and benefits eligibility
• Strong communication, family meeting facilitation, and interdisciplinary collaboration skills
• Ability to perform data abstraction and determine medical necessity per guidelines
