Roles & Responsibilities
- Coordinate patient care across the continuum using strong care coordination and discharge planning skills
- Conduct utilization review and prior authorization activities to support appropriate care and timely transitions
- Assess needs and order DME when necessary to ensure safe discharges
- Collaborate with interdisciplinary team members including social services to resolve barriers to discharge
- Manage appeals and denials, and support benefits eligibility processes
- Document thoroughly and accurately in Epic to maintain compliance and continuity of care
- Support worker's compensation case management processes and follow protective services procedures when indicated
Qualifications
- Minimum 2 years of acute hospital case management experience (current within the last year)
- Experience with Epic charting required
- Travel experience required; first-time travelers may be screened with Clinical approval
- Experience with care coordination, discharge planning, utilization review, prior authorizations and DME needs assessment
- Familiarity with HMOs/IPAs, CMS/DRG, HIPAA, The Joint Commission/core measures, and worker's compensation processes
- Ability to float between units and manage a caseload (up to ~25) across age groups
Knowledge & Skills
- Epic electronic health record proficiency
- Strong knowledge of discharge planning and care coordination best practices
- Skilled in utilization review, admission criteria, appeals and denials
- Understanding of benefits eligibility and prior authorization workflows
- Knowledge of advance directives/POLST and protective services processes
- Familiarity with regulatory standards including CMS, DRG and The Joint Commission
