Roles & Responsibilities
- Conduct thorough initial and ongoing client assessments to identify medical, functional, and psychosocial needs
- Develop and update individualized care plans in collaboration with nurses, physicians, caregivers, and families
- Coordinate and manage caregiver schedules to ensure consistent, high-quality coverage
- Match caregivers (CNA/HHA) to clients based on skills, preferences, and compatibility
- Onboard, supervise, and coach caregiving staff to maintain safety and service quality
- Serve as the primary point of contact for clients and families, responding to questions and changes promptly
- Perform regular home visits to monitor care quality, client satisfaction, and caregiver performance
- Educate families on services, home safety, and care plan expectations while ensuring regulatory compliance
Qualifications
- Previous experience coordinating home care services or managing caregiver schedules preferred
- Clinical understanding of basic care needs and ability to read and act on care plans
- Strong communication, organizational, and problem-solving skills
- Comfort with conducting home visits and interacting compassionately with clients and families
- CPR/First Aid certification preferred
- Valid driver's license and reliable transportation preferred for in-home visits
- Ability to pass background checks and any required screenings
Education
- High school diploma or equivalent required
- Associate degree or healthcare-related coursework (nursing, social work, or related) preferred
Knowledge & Skills
- Care planning and client assessment expertise
- Caregiver management and scheduling acumen
- Clinical understanding combined with administrative skills
- Focus on care continuity, service quality, and patient satisfaction
- Familiarity with regulatory compliance in home health settings
- Excellent communication and relationship-building abilities
