Roles & Responsibilities
- Conduct thorough initial and ongoing client assessments to identify medical, functional, and psychosocial needs
- Develop and update individualized care plans in collaboration with nurses, caregivers, physicians, and family members
- Coordinate and manage caregiver schedules to ensure consistent, reliable care delivery
- Serve as the primary liaison between clients, families, caregivers, and healthcare providers to promote care continuity
- Monitor service quality and patient satisfaction and take action to address concerns
- Ensure regulatory compliance and documentation standards are met for safe, effective care
- Adjust care plans proactively as client needs evolve to maintain safety and goal alignment
- Support caregiver management including onboarding, communication, and performance follow-up
Qualifications
- Experience in client assessment, care planning, or caregiver coordination (healthcare or social services preferred)
- Strong communication and interpersonal skills to engage clients, families, and clinical teams
- Familiarity with regulatory compliance and documentation best practices
- Proven organizational and scheduling abilities to manage multiple clients and caregivers
- Comfort working in home-based care settings and collaborating with interdisciplinary teams
- Valid driver's license and reliable transportation preferred for in-home visits
- CPR certification or other relevant certifications are a plus
- Willingness to participate in training and continuing education
Education
- High school diploma or equivalent required
- Associate degree or Bachelor's degree in nursing, social work, healthcare administration, or related field preferred
Knowledge & Skills
- Care continuity and service quality principles
- Patient satisfaction focus and relationship building
- Care planning and client assessment techniques
- Caregiver scheduling and management practices
- Clinical understanding sufficient to coordinate with nurses and physicians
- Administrative skills and attention to documentation detail
- Understanding of regulatory requirements in home care settings
- Strong verbal and written communication skills
