Roles & Responsibilities

• Provide medical direction and clinical oversight for PACE participants, ensuring high-quality, person-centered care
• Lead and supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs) to maintain excellent clinical standards
• Oversee and drive the Quality Assurance Performance Improvement (QAPI) program to improve outcomes
• Develop, maintain and implement medical policies, procedures and clinical protocols
• Perform history & physical exams, develop treatment plans, and coordinate care across settings where participants live
• Serve as an active interdisciplinary team member—informing care plans and advising on diagnoses, function, and treatment options
• Educate participants and families about health maintenance and chronic disease management to support informed decision-making
• Interview and evaluate applicants and caregivers for the PACE program and participate in intake/assessment decisions
• Collaborate with nursing and other team members to supervise and guide participant treatment programs
• Participate in provider on-call rotation and occasional regional travel as required

Qualifications

• Board Certified in Family Medicine or Internal Medicine (explicit requirement)
• Fellowship in Geriatric Medicine or Certificate of Added Qualifications (CAQ) in Geriatric Medicine (required/preferred as listed)
• Minimum of 1 year experience working with frail or elderly populations (explicitly required)
• Willingness to serve as Medical Director and provide medical supervision and leadership
• Driver’s license, proof of insurance, and reliable transportation for periodic regional travel (explicitly required)
• Ability to participate in provider on-call rotation
• Not accepting visa candidates (explicitly stated); must be eligible to work without visa sponsorship

Education

• MD or DO with Board Certification in Family Medicine or Internal Medicine
• Fellowship training in Geriatric Medicine or CAQ in Geriatric Medicine (as stated in job posting)

Knowledge & Skills

• Expertise in geriatrics and chronic disease management for frail older adults
• Strong skills in physical assessment and conducting history & physical exams
• Experience with Quality Assurance Performance Improvement (QAPI) processes
• Leadership and interdisciplinary team-based care experience
• Proficiency with Microsoft Office (Word, Excel, PowerPoint) and standard email/calendaring systems
• Excellent organizational, customer service, flexibility, and problem-solving abilities
• Ability to develop medical policies, protocols and provide clinical oversight
