Roles & Responsibilities
- Provide medical direction and clinical oversight for participant care across the PACE program
- Conduct history and physical examinations for new participants and perform event-based and periodic reassessments
- Develop individualized medical treatment plans and coordinate care across settings
- Supervise and mentor Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)
- Lead and participate in the interdisciplinary team, contributing medical expertise to care planning
- Oversee Quality Assurance Performance Improvement (QAPI) activities and drive clinical outcomes
- Develop and maintain medical policies, procedures, standards and protocols
- Order and evaluate laboratory and diagnostic tests and document findings systematically
- Participate in intake and assessment decisions and communicate prognosis and care options with participants and caregivers
- Collaborate with nursing and other team members to implement and supervise treatment programs
Qualifications
- MD or DO degree (required) with valid medical licensure in Massachusetts or eligibility to obtain licensure
- Clinical experience in geriatrics, primary care or chronic disease management for older adults
- Demonstrated ability to supervise and provide direction to PCPs and APPs
- Experience with clinical quality programs (QAPI) and policy/procedure development
- Comfort participating in an interdisciplinary team and engaging with participants and families
- Willingness to participate in provider on-call rotation as needed
- Desire to work in an alternate care setting focused on keeping seniors safely in the community
Education
- MD or DO required
- Board certification or eligibility preferred (e.g., Geriatrics, Family Medicine, Internal Medicine)
Knowledge & Skills
- Strong skills in physical assessment and chronic disease management for frail older adults
- Experience with medical policy development and quality assurance practices
- Ability to develop and implement care plans across multiple care settings
- Excellent communication and team leadership skills
- Competence ordering and interpreting labs and diagnostic studies
- Skilled at patient and family education and shared decision-making
- Familiarity with interdisciplinary care coordination and intake/assessment processes
