Roles & Responsibilities

• Provide compassionate direct patient care to residents in a long term care setting
• Administer medications and treatments accurately and safely
• Monitor and report changes in patient condition to the interdisciplinary team
• Document patient assessments, interventions, and outcomes in the chart
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Assist residents with activities of daily living and promote dignity and independence
• Participate in shift handoffs and ensure smooth continuity of care

Qualifications

• Current LPN license (practical nursing license) required
• CPR/BLS certification required or willing to obtain
• Ability to work nights and every other weekend as noted
• Comfortable with medication administration, charting, and hands-on resident care
• Strong communication skills and a team-oriented attitude

Education

• LPN diploma or certificate
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and basic nursing procedures
• Skilled in resident assessment and documentation
• Familiarity with infection control practices
• Basic wound care and ADL assistance
• Good time management and prioritization during busy shifts
• Ability to use electronic medical records and standard nursing documentation
