Roles & Responsibilities

• Provide compassionate direct patient care including ADLs and monitoring
• Administer medications and treatments safely and accurately
• Perform focused patient assessments and report changes promptly
• Document care thoroughly in the patient record and communicate with the team
• Collaborate with RNs, physicians, and support staff to coordinate care plans
• Support patients and families with clear, respectful education and guidance
• Maintain a safe environment by following policies, infection control, and safety protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license — New Mexico license required or eligible
• Current BLS (Basic Life Support) certification required
• Prefer 1+ years of recent LPN experience in long-term care or similar setting
• Ability to pass background check, drug screen, and required health screenings
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing program; LPN licensure required

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Comfortable with wound care, IV site observation, and basic assessments
• Familiarity with electronic medical records and accurate charting
• Excellent interpersonal and patient education skills
• Good time management and ability to prioritize in a fast-paced environment
