Roles & Responsibilities

• Assess and evaluate patients in their homes to create individualized treatment plans
• Deliver evidence-based care focusing on orthopedics, neurologic rehabilitation, post-operative recovery, and body mechanics
• Develop and teach personalized home exercise programs to maximize patient independence
• Document thoroughly and timely using OASIS, EPIC (remote laptop), and Rover (phone app)
• Provide clear patient and family education to promote safety and functional progress
• Manage an efficient caseload of up to 6 visits per day while maintaining quality of care
• Collaborate with interdisciplinary team members and communicate care plans with providers and families
• Maintain required certifications and adhere to facility vaccination and safety policies

Qualifications

• Valid physical therapy state license or pending license accepted for this role
• Minimum 1 year of skilled home health experience required; first-time travel clinicians welcomed
• BCLS (Basic Cardiac Life Support) certification required
• Comfortable using EPIC, Rover, and completing OASIS documentation
• Reliable transportation to travel to patient homes and willingness to manage occasional drives up to roughly 60 miles in a day
• Up-to-date primary COVID vaccination series plus one booster as required by facility

Knowledge & Skills

• Strong knowledge of Home Health best practices and OASIS documentation
• Skilled in assessment, patient education, and developing Home Exercise Programs
• Experience with orthopedics, post-operative care, and neurologic rehabilitation
• Good command of body mechanics, mobility training, and fall prevention strategies
• Comfortable with EPIC remote charting and Rover mobile documentation
• Ability to manage time, productivity expectations (around 30 points), and up to 6 visits per day
