Shift Type: Monday-Friday (5x8 schedule), 8:00 AM - 4:30 PM, Day shift; every-other-weekend required (weekend rotation)
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate patient care across interdisciplinary teams to ensure smooth transitions from hospital to home or post-acute settings
- Perform discharge planning and utilization review to help patients get the right level of care at the right time
- Conduct needs assessments and independently arrange DME, SNF placements, and ambulance transport when required
- Lead and participate in family meetings to develop patient-centered plans of care
- Document thoroughly in Epic and other computerized systems to maintain clear, compliant records
- Facilitate prior authorizations and benefits eligibility checks with HMOs/IPAs and payers
- Collaborate with Physical Therapy and Social Services to support patient outcomes
- Apply knowledge of CMS, DRG, and The Joint Commission standards to daily case management decisions
Qualifications
- Active CA state license required — pending license accepted during processing but must be active by start date
- Minimum 2 years of Acute Care Case Management / Discharge Planning experience (hospital experience required; behavioral health does not apply)
- Travel assignment experience required — seasoned traveler preferred (this role is not open to first-time travelers)
- Proven experience with Epic and computerized documentation systems (charting required)
- Comfort managing high patient ratios (experience with ~1:25 caseload)
- Ability to work every-other-weekend to meet departmental needs
- Strong communication and family-meeting facilitation skills
- Familiarity with prior authorizations, benefits eligibility, HMOs/IPAs, and coordinating post-acute care

- Experience ordering and arranging DME, SNF placements, and ambulance transport
- Working knowledge of CMS, DRG, and The Joint Commission standards
- Comfort with Emergency Department, ICU, Medical-Surgical and Surgical Services workflows
- Skilled in Epic charting and computerized documentation
- Ability to organize and participate in family meetings and develop plans of care
