Roles & Responsibilities
- Evaluate and treat residents with speech, language, cognitive-communication and swallowing disorders in the SNF setting
- Develop individualized treatment plans and document progress according to facility and Medicare guidelines
- Collaborate with nursing, therapy colleagues, and families to support resident-centered goals
- Provide clear, timely documentation and discharge recommendations
- Educate staff and caregivers on strategies to support carryover and safety
- Adapt interventions to meet the needs of an older adult population in a skilled nursing environment
Qualifications
- Active state SLP license (required)
- 1–3 years of clinical experience in skilled nursing or related settings preferred
- Comfortable working PRN shifts and adjusting to facility-specific workflows
- Strong documentation skills and familiarity with SNF/long-term care processes
- Current CPR certification (or willingness to obtain) and background clearances as required by facility
Education
- Master’s degree in Speech-Language Pathology or equivalent (typical requirement for SLP roles)
Knowledge & Skills
- Skilled in dysphagia management, cognitive-communication therapy, and language rehabilitation
- Good clinical reasoning and individualized treatment planning
- Excellent interpersonal and communication skills for working with residents and interdisciplinary teams
- Proficient documentation practices and understanding of SNF regulatory expectations
- Ability to manage a varied caseload and prioritize tasks in a busy facility
