Roles & Responsibilities
- Provide compassionate, evidence-based in-home physical therapy to adults and seniors
- Assess patient function and develop individualized treatment plans that improve mobility and independence
- Deliver skilled therapeutic interventions and adapt techniques to each home environment
- Document patient progress clearly and timely in the clinical record
- Educate patients and caregivers on safe mobility, home exercise programs, and fall prevention
- Collaborate with nurses, case managers, and the interdisciplinary team to support holistic care
- Manage a flexible caseload and adjust plans as patient needs evolve
Qualifications
- PT or DPT (position lists PT and DPT as required skills)
- All experience levels welcome — any experience, including new graduates
- Comfortable practicing in a home health setting with frequent floating between clients
- Eligibility to practice in the state of Missouri (hold or be able to obtain required state credentials)
Education
- Doctor of Physical Therapy (DPT) or equivalent (PT credentials listed)
Knowledge & Skills
- Strong clinical assessment and treatment planning skills
- Hands-on therapy techniques and safe patient handling
- Excellent verbal communication and patient education skills
- Time management and organizational skills for visit-based scheduling
- Ability to document thoroughly and accurately
