Roles & Responsibilities

• Provide compassionate, competent LPN care to residents at Casa Del Sol Center
• Administer medications and treatments accurately and document per facility policy
• Monitor patient status and report changes to the interdisciplinary team
• Assist with admission, transfer, and discharge processes to ensure continuity of care
• Collaborate with nurses, therapists, and support staff to deliver person-centered care
• Educate residents and families about care plans and follow-up instructions
• Maintain accurate clinical documentation and adhere to infection control standards

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in New Mexico
• Basic Life Support (BLS) certification required; additional certifications a plus
• Previous LPN experience in a long-term care or skilled nursing setting preferred
• Strong communication skills and ability to work as part of a care team
• Current immunizations and ability to meet facility onboarding requirements

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Solid knowledge of medication administration and charting practices
• Skilled in resident assessment, wound care basics, and vitals monitoring
• Comfortable using electronic health record systems for documentation
• Good time management and prioritization in a fast-paced clinical setting
• Empathy, professionalism, and strong patient advocacy skills
