Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Administer medications and treatments safely, following facility policies
• Monitor and document patient vital signs, status changes, and care provided
• Collaborate with RNs, providers, and interdisciplinary team members to support care plans
• Perform wound care, catheter care, and other skilled nursing tasks as assigned
• Maintain accurate, timely charting in the electronic medical record
• Educate patients and families about care plans, medications, and discharge instructions
• Participate in infection control practices and maintain a safe care environment

Qualifications

• Active LPN license eligible for practice in New Mexico
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Graduation from an accredited Practical Nursing program
• Ability to work evening hours (2:00 PM - 10:00 PM) and adapt to a contract assignment
• Current immunizations and TB screening per facility requirements
• Strong communication skills and a team-oriented approach

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care, catheter management, and ADL support
• Familiarity with electronic medical records and accurate charting
• Patient assessment, vital sign monitoring, and early recognition of changes
• Time management, prioritization, and working effectively in a clinical team
• Professional, empathetic communication with patients, families, and staff
