Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at bedside
• Administer medications and treatments accurately and safely according to orders
• Assess and monitor patients, documenting changes and communicating concerns promptly
• Collaborate with nurses, providers and support staff to coordinate excellent care
• Help with admissions, discharges and transitions to ensure continuity of care
• Educate patients and families about care plans, medications and follow-up
• Maintain accurate, timely clinical documentation in patient records
• Support infection control and safety practices to protect patients and staff

Qualifications

• Current, active Licensed Practical Nurse (LPN) license (required)
• Previous LPN experience preferred; experience in long-term care or similar settings a plus
• Basic Life Support (BLS/CPR) certification preferred
• Ability to pass background check and drug screening as required for travel assignments
• Comfort working as part of a multi-disciplinary team and supporting a positive culture

Education

• Graduate of an accredited practical nursing or vocational nursing program
• LPN certificate or diploma required

Knowledge & Skills

• Strong clinical assessment and patient-care skills
• Medication administration and safe documentation practices
• Familiarity with resident-centered care and long-term care workflows
• Clear, compassionate communication with patients, families and team members
• Good time management, prioritization and the ability to work in a fast-paced setting
• Basic electronic health record charting and accurate record keeping
