Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse
• Administer medications and treatments safely and accurately
• Monitor patient condition, record vital signs, and report changes to the care team
• Document care in the medical record clearly and timely
• Collaborate with nurses, therapists, and physicians to implement care plans
• Support residents and families with education, comfort, and dignity

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice or eligibility to obtain one
• Basic Life Support (BLS/CPR) certification (or willingness to obtain prior to start)
• Comfortable working in a long-term care or skilled nursing environment
• Strong communication skills and ability to work well within a multidisciplinary team
• Professional, compassionate approach to resident-centered care

Education

• Completion of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Medication administration and safe nursing practices
• Patient assessment and monitoring of vital signs
• Clinical documentation and electronic medical record familiarity
• Infection prevention and standard precautions
• Ability to prioritize tasks and respond to changing clinical needs
