Roles & Responsibilities

• Provide compassionate, patient-centered care to residents at Casa Del Sol Center
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, patient status, and care plans in charts
• Collaborate with RNs, CNAs, therapists, and interdisciplinary team members to deliver coordinated care
• Educate residents and families on routine care, medication instructions, and safety
• Participate in quality improvement initiatives and follow facility policies to maintain high standards

Qualifications

• Active LPN license (Licensed Practical Nurse) — required
• Current BLS/CPR certification or ability to obtain prior to start
• Ability to pass background and health screenings per facility policy
• Comfortable working in a long-term care or skilled nursing environment — experience preferred
• Strong communication skills and a collaborative attitude

Education

• Vocational nursing diploma or equivalent completion of an accredited LPN program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control principles
• Accurate vital signs assessment and patient observation
• Familiarity with electronic health record documentation is a plus
• Time management, prioritization, and teamwork in a busy care setting
