Roles & Responsibilities
- Provide compassionate direct patient care and prioritize patient safety every shift
- Administer medications and treatments accurately while following facility protocols
- Monitor and document vital signs, patient status changes, and reporting to RN/MD promptly
- Perform wound care and assist with basic clinical procedures as appropriate
- Collaborate with the interdisciplinary care team to create and follow care plans
- Educate patients and families about care, medications, and discharge instructions
- Maintain clear, timely electronic charting to support continuity of care
- Support admissions, transfers, and safe discharges with professionalism and empathy
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain one
- Current BLS (Basic Life Support) certification required
- Comfortable working the 2:00 PM - 10:00 PM shift and adapting to a fast-paced environment
- Strong clinical judgment, reliable documentation skills, and excellent communication
- Prior long-term care or skilled nursing experience is a plus, but new grads with strong clinical training are welcome
Education
- Completion of an accredited Practical Nursing program (LPN diploma or certificate)
- High school diploma or GED required

- Medication administration and medication safety practices
- Basic patient assessment and monitoring of vital signs
- Wound care and infection prevention techniques
- Electronic health record charting and accurate documentation
- Clear, compassionate patient and family communication
