Roles & Responsibilities

• Provide compassionate direct patient care and prioritize patient safety every shift
• Administer medications and treatments accurately while following facility protocols
• Monitor and document vital signs, patient status changes, and reporting to RN/MD promptly
• Perform wound care and assist with basic clinical procedures as appropriate
• Collaborate with the interdisciplinary care team to create and follow care plans
• Educate patients and families about care, medications, and discharge instructions
• Maintain clear, timely electronic charting to support continuity of care
• Support admissions, transfers, and safe discharges with professionalism and empathy

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain one
• Current BLS (Basic Life Support) certification required
• Comfortable working the 2:00 PM - 10:00 PM shift and adapting to a fast-paced environment
• Strong clinical judgment, reliable documentation skills, and excellent communication
• Prior long-term care or skilled nursing experience is a plus, but new grads with strong clinical training are welcome

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)
• High school diploma or GED required

Knowledge & Skills

• Medication administration and medication safety practices
• Basic patient assessment and monitoring of vital signs
• Wound care and infection prevention techniques
• Electronic health record charting and accurate documentation
• Clear, compassionate patient and family communication
• Team collaboration and following standing orders and care plans
