Roles & Responsibilities

• Provide compassionate direct nursing care to residents in the skilled care setting
• Administer medications and treatments accurately and safely according to orders
• Assess patient status, monitor changes and report findings to the interdisciplinary team
• Document care precisely in the medical record to ensure continuity and compliance
• Collaborate with nurses, therapists and support staff to create person-centered plans
• Educate patients and families about care plans, medications and follow-up needs
• Maintain infection control standards and a safe, respectful environment for residents

Qualifications

• Current LPN license (Licensed Practical Nurse) valid in New Mexico
• Completion of an accredited practical/vocational nursing program
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Current healthcare clinical experience preferred; new grads with strong clinical skills considered
• Ability to pass background check, drug screen and meet facility immunization requirements

Education

• Graduate of an accredited Practical/Vocational Nursing program (diploma or certificate)

Knowledge & Skills

• Confident medication administration and documentation skills
• Strong clinical assessment and observation abilities
• Comfortable using electronic health records for charting
• Clear, compassionate communication with patients, families and team members
• Time management and prioritization in a fast-paced care environment
• Knowledge of infection control, safety protocols and resident-centered care
