Roles & Responsibilities

• Provide compassionate direct patient care to residents at Casa Del Sol Center
• Administer medications and treatments accurately and document in the chart
• Perform routine assessments, record vital signs and report changes promptly
• Collaborate with the care team to develop and follow individualized care plans
• Assist with activities of daily living while promoting resident dignity and independence
• Maintain a safe, clean environment and follow infection control protocols
• Communicate clearly with families and team members to ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) license - New Mexico licensure required or compact eligibility where applicable
• Graduate of an accredited LPN/Vocational Nursing program
• Current Basic Life Support (BLS) certification preferred or obtainable upon hire
• Ability to pass required background check and drug screening
• Professional attitude, strong communication skills and a team-first mindset
• Previous long-term care or geriatric experience preferred

Education

• Graduate of an accredited LPN/Vocational Nursing program, required

Knowledge & Skills

• Medication administration and accurate documentation
• Basic wound care and dressing changes
• Patient assessment, vital signs and reporting clinical changes
• Electronic health record (EHR) charting and documentation best practices
• Infection prevention, isolation precautions and safety protocols
• Effective communication with residents, families and interdisciplinary teams
